Most accidents occur when work behaviour, equipment or environment are taken for granted, and the system fails unexpectedly. Accident investigations determine how and why these failures occur, and how the findings can prevent repeat incidents. Students are taught to conduct accident investigations with prevention in mind, not to assign blame or criticize a workplace. Language options: English, French, Spanish
